Passport Clinic in Virden

On Saturday, December 1st I am hosting a Passport Clinic at the Sacred Heart Roman Catholic Church in Virden from 10:00am to 2:00pm.  Come to this one-stop shop where staff will be available to assist you in either renewing or becoming a first time passport holder.  A photographer will also be available for your passport photos and my office will mail your passport application on your behalf.  

I truly believe in improving constituent services for the people of Westman. I also want to make it as easy as possible for residents to access the services of the Government of Canada and be available to speak to any of your concerns.

For Adult General Passport Applications (first time applicants and for individuals with passports that have been expired more than one year), you MUST bring:

1. An application (signed by your guarantor - a person with a valid Canadian passport who has known you for at least two years and is at least 18 years of age)
2. Your original Birth Certificate or Canadian Citizenship Card/Certificate
3. A copy (front and back) of your Driver's Licence or Government issued photo ID - the photocopy must be signed and dated by your guarantor
4. Two identical passport photos taken within the last 6 months (available on site) - the back of your stamped photo must be signed by your guarantor 
5. A copy of any government issued legal documents that support a change in name (i.e. marriage certificate)
6. Old passport (if applicable)

For Simplified Renewal Applications, you MUST bring:

1. An application (including contact/mailing information for two references that have known you for at least two years and are not family members)
2. Two identical passport photos taken within the last 6 months (available on site)
3. Current passport

For Child Passport Applications, you MUST bring:

1. An application (signed by your guarantor - a person with a valid Canadian passport who has known you for at least two years and is at least 18 years of age)
2. Your child's original Long-Form Birth Certificate or Canadian Citizenship Card/Certificate
3. Two identical passport photos of the child taken within the last 6 months (available on site) - the back of the stamped photo must be signed by your guarantor
4. Current passport (if applicable)

There will be photographers on site and staff to help you with your application!

Cash or debit is NOT accepted
$120 - 5-year adult passport (16 or over)
$160 - 10-year adult passport (16 or over)
$57 - child passport (0-15 years of age)

For Pictures ($25 taxes included)
Cash or cheque only (no debit or credit card)
